Formulation
This assessment is suitable if it is your first visit or if you are returning for a new episode of care where your circumstances have changed.
Available for adults aged 18 and over
Discussion focuses on

Understanding current concerns in the context of your unique life-experience with the intention of seeking clarity (formulation and if relevant a diagnosis)

Opportunity to consider possible treatment options (medication and psychological therapy)

Resources available locally and nationally to support you

How to access care in a crisis or emergency

The next steps in your care
Progress Reviews
At the time of the diagnostic appointment, a thorough and compassionate assessment discussion would have helped determine the most appropriate next steps for your care.
Clients who have completed their initial diagnostic review will be offered treatment review sessions within this service if appropriate and a shared care agreement is in place.
A shared care agreement will be arranged with the client's GP by mutual agreement.
Supportive Listening/
Coaching Sessions/Education
This is a recovery focused service promoting self efficacy and autonomy. There is a recognition of the need for self-individuation and a greater understanding of individual experience.
Clients who wish to have a safe confidential space in which to discuss their thoughts and feelings to achieve clarity and solutions around their dilemmas are welcome to book a session.
These sessions are available to clients as well as their carers.
