How we use your data

This service is registered with the Information Commissioner's Office ​to ensure commitment to safe processing of data and undertakes regular self-audit of data processing securtiy compliance.

The information that you provide including personal details, health related information and next of kin details will only be used for the purpose of providing clinical care to you. In the event of concern for your immediate safety or the safety of others some of this information including your name,contact details and the nature of the imminent concern may be shared with other agencies to prevent harm coming to you or any other individual. You will always be informed if such a concern exists.

When recording information about your health and your condition this will be kept to a minimum and relevant to providing care to you.

You are welcome to discuss any questions you may have and make clinicians aware of your specific concerns and requests around the information written in your medical record.
You will also have the opportunity to share information and access to the client portal with your carer if you so wish.

Consent
On registration with the Carepatron portal you will receive consent forms requesting your expressed consent to assessments (in person or video conferencing), storage of your medical records, contacting your emergency contact and your GP. Please get in touch if you would like to arrange a telephone call to discuss any of this further prior to booking or registering on the Carepatron portal.

Contact Form
All health related information is stored using the Carepatron medical record keeping system.
The contact form is not secure so should only be used to express an interest in booking further and share contact details to arrange a telephone call. This initial call would be to answer any questions about the assessment process. All confidential medical details may be discussed and recorded during the consultation held there after if the client chooses to go ahead with booking an initial assessment.
Information shared using the contact form will not be stored and used only to make contact for the purpose of providing information required to prospective clients considering an initial assessment.

Data Breaches
In the unexpected event of a Data Breach we will contact you to arrange a time to discuss the breach in order to provide full details of the breach, how the breach will be resolved and how you may take any concerns you have forward.

Carepatron
Your medical records are stored on a system called Carepatron. You will have access to this information via a secure patient portal. You may read about their privacy policy here and their medical compliance policy here

Your payment details are stored securely on the Carepatron portal are processed via there secure platform.

Communicating with your GP
A letter will be prepared for your GP with your agreement to arrange shared care, following your assessment or review and is inclusive of the appointment fee. The letter will detail a collaborative care plan agreed during the session. If shared care is not part of your care plan no direct communication will be made with your GP unless requested by you. You may however choose to share your letter with your GP, carer's or other agencies.
Reports for other agencies
Clients sometimes require a detailed report that answers a specific question, that may relate to fitness for work, disability application, mental capacity to name a few. Fees related to reports are determined separately based on an hourly fee.
